2. Filing a DMCA Notice (Takedown Request)
To be effective, your infringement notice must include all of the following:

A physical or electronic signature of the copyright owner or a person authorized to act on their behalf.

Identification of the copyrighted work claimed to have been infringed (or a representative list of works if multiple works are involved).

Identification of the infringing material and information sufficient to permit USPA to locate the material (such as a specific URL).

Contact information of the complaining party, including name, address, phone number, and email address.

A statement that the complaining party has a good faith belief that use of the material is not authorized by the copyright owner, its agent, or the law.

A statement that the information in the notification is accurate, and under penalty of perjury, that the complaining party is authorized to act on behalf of the copyright owner.

3. Counter-Notification
If you believe that material you submitted was removed or disabled by mistake or misidentification, you may submit a counter-notification to our DMCA Agent. Your counter-notice must include:

Your physical or electronic signature.

Identification of the material that has been removed or disabled and the location where it appeared before removal.

A statement under penalty of perjury that you have a good faith belief that the material was removed or disabled due to mistake or misidentification.

Your name, address, phone number, and email address.

A statement that you consent to the jurisdiction of the Federal District Court for the judicial district of your address, or if outside the United States, to the jurisdiction of any judicial district in which USPA may be found, and that you will accept service of process from the person who filed the original DMCA notice or their agent.
Upon receiving a valid counter-notification, USPA will provide the original complainant with a copy. Unless the copyright owner files a legal action within 10–14 business days, USPA may restore the removed material.
4. Repeat Infringers
USPA reserves the right to terminate, suspend, or revoke the membership and/or account of any user who is deemed to be a repeat infringer.
